Description
* Offers physical, emotional, social, and spiritual support to individuals with a life limiting illness* Support is also given to family members both before and up to thirteen months after the death* Works on pain and symptom control to improve the quality of the final days and weeks in the patients life* Services are provided in the patients home, home of a family member, or in area nursing homes * Services are available for patients with special needs in the Columbus Hospice House, an 18-bed inpatient hospice facility

Hours: Office: 8AM-5PM, Mon-Fri; on call: 24
hours, 7 days a week
